各位教職員工及同學 您好！中央疫情指揮中心表示，COVID-19本土個案活動範圍遍及全台多各縣市，故請全校教職員工生協助填報問卷，俾利校方掌握師生之健康狀況。並請落實自主健康管理14天（上班、上課全程配戴口罩；切勿聚餐、共食），如有疑似症狀，請儘速戴口罩就醫，主動告知醫師活動接觸史，另請通報衛保組（03-2804814）或校安中心（03-2805666）。

Central Epidemic Command Center (CECC) announced confirmed indigenous case of coronavirus disease 2019 (COVID-19) in Taiwan.Members of the public who have visited those locations during the time periods mentioned below ，please fill in the Questionnaire Survey and are reminded to practice self-health monitoring for 14 days. If suspected symptoms develop, members of the public must put on a mask and voluntarily inform the physician of any history of contact and activity.
